I am a provider for:

• Evernorth Behavioral Health (formerly Cigna)
• Molina Healthcare
• United Behavioral Health (UBH)
• Optum Behavioral Health
• United Medical Resources (UMR)

If you have health insurance for which I’m not a provider, you will need to pay the customary fee at the time of service. However, I can provide a “superbill” that lists treatment received and other information needed for you to seek reimbursement from your insurance company.

Fees

My usual and customary fee is $150 per 50-minute session, although I reserve the right to periodically adjust this fee. You will be notified of any fee adjustment in advance. In addition, this fee may be adjusted by contract with insurance companies, managed care organizations, or other third-party payors, or by agreement with me.

Si usted tiene un seguro de salud para el cual no soy proveedora, usted tendrá que pagar la tarifa habitual en el momento de servicio. Sin embargo, puedo proporcionarle un “superbill” que enumera el tratamiento recibido y otra información necesaria para que usted pueda obtener el reembolso de su compañía de seguros.

Honorarios

Mi tarifa habitual es de $150 por sesión de 50 minutos, aunque reservo el derecho de ajustar esta tarifa periódicamente. Se le notificará por adelantado de cualquier ajuste de la tarifa. Además, esta tarifa puede ser ajustada por contrato con las compañías de seguros, organizaciones de atención administrada, u otros terceros pagadores, o por acuerdo conmigo.
